If the declaration of compliance with regulations is based on the self-assessment results of the organization or individual (first party), then the declaration of compliance with regulations includes:
a) Regulation conformity report (according to Form No. 2.CBHC/HQ specified in Appendix III Circular No. 28/2012 / TT-BKHCN and additional content specified in Article 7 No. 1 of this Circular);
b) The self-assessment report includes the following information:
